2019 Ohio National Specialty Health Screenings-One form per dog-Please submit before checking out

A limited number of appointments and test kits are available; pre-registration is required. Time and schedule permitting, walk-in appointments may be available. Please do not rely on walk-in appointments to be offered; these are at the discretion of the veterinarian providing the service.

Screenings will be located in the Call of the Wild meeting rooms. Sign-In will be at the desk in the Tecumseh Falls lobby. CAER eye appointments should arrive at least 20 minutes prior to their appointment time; all other appointments should arrive at least 10 minutes prior to their appointment time. Some testing will be available throughout the week in the ballroom. Pre-registered appointments will have applicable OFA forms completed for them Yes! This means you will not have to perform this tedious task or remember to bring your dog’s AKC and microchip info with you! For walk-ins, a limited number of blank forms will be available at the check-in desk. Upon arrival, please request your dog’s health clinicpacket, sign the enclosed OFA forms and make your dog available for microchip scanning for the staff to confirm accuracy and identity.

Please remember, the veterinarians and vet nurses, as well as RRCUS club members volunteering at this year’s healthclinic are providing a service to you. Those participants acting in a rude/disrespectful manner or using obscene/abusive language will be asked to leave, and no refunds will be offered to those individuals.

Appointment times will be scheduled after the Judging Program has been made available.
Remember to indicate if the dog you are presenting for health testing will be showing that day and what class(s) or event(s) they are entered.

Payment:

Payment should be made by credit card via RRCUS Store or check payable to RRCUS and mailed with your health clinic registration form to the address above.
Payment and health clinic registration forms must be received no later than September 6th, 2019. Registrations received after September 6th, 2019 will be subject to walk-in prices. If mailing a check, please include your dog’s callname in the memo box. Returned checks will be subject to a returned check fee of $50.

Should walk-in appointments be available the day of the event, payment to be made by cash or check only.Thank you for helping produce healthier puppies through health-tested parents!

Each clinic has a minimum registration requirement. Updates will be made on the event’s Facebook page as to when

the minimum number of participating dogs has been reached or if more participants are needed.

In the event of particular health test being cancelled, those pre-registered for that particular testing will be contacted

and payment for that health screening will be refunded as soon as possible.

Appointments that are no-show at the health screening event will not receive a refund.

OFA Congenital Cardiac Database Screening - Auscultation

Minimum Required: 35

OFA Companion Animal Eye Registry (CAER) Screening

Minimum Required: 50

Clinic Hours: 10AM 2PM; Services provided by: Margaret Foss, DVM, DACVO

Additional Information About This Test:
Please arrive 20 minutes prior to your dog’s appointment for eye drops to be administered.

OFA Thyroid Database Screening Grant Supported!
Minimum Required: 100 (Please note, MSU has a grant covering the testing cost, but this is only available to us if we reach the 100 pre- registered samples required!)

Clinic Hours: 10AM 2PM, Tuesday through Saturday
Fees: Pre-registration: $15 (this INCLUDES the OFA Application for Thyroid Database application fee!)

Additional Information About This Test:
Blood will be collected for this test. You are welcome to step out of the room, if necessary.
If possible, please fast your dog at least 6 hours prior to sample collection.

Because it is not guaranteed a dog who tests negative at any one time will not develop thyroiditis later on in its life, OFA recommends annual thyroid testing for the first 4 years, then every other year thereafter.

DNA-Based Screenings:

AKC DNA Registry

Minimum Required: None

Rhodesian Ridgeback Inherited Arrhythmia (RR IVA) DNA Screening

Minimum Required: None

Fees: Pre-registration: $48
Walk-in available at same price as pre-registration (cash or check only)

Embark Comprehensive DNA Testing

Thanks to a generous donation from Mary Ann Mauer, the first 25 tests will receive $10 off!

Minimum Required: 20

Fees: Pre-registration w/ minimum reached: $145 (regularly $180!) If minimum is not reached test kits will be $160 each

HomeAgain Microchip

Minimum Required: None

Fees: Pre-registration: $25 (includes life-long chip registration and registry in the HomeAgain National Pet Recovery Database, as well as first year of HomeAgain Membership enrolment.)
Walk-in (if additional chips are available): $30
Additional Information:

It is recommended your dog have permanent ID, such as a microchip, prior to having their health screenings preformed; this allows the examining veterinarian to verify the identity of your dog on the OFA form.
